Cannot Obtain RMA

My three-month-old DTC920 Canvio Red 2TB 5400 drive failed. In trying to obtain an RMA, I kept finding myself back to square one when I click the Continue button on the attached RMA application.
I'm responsible for ordering hundreds of Toshiba drives over the years, and I'm very disappointed with this experience.

I finally received an RMA by making a telephone call. If calling is the only option, then I think the online option for requesting an RMA should be eliminated.
